The item listed is made of stainless steel with the 3D urethane encased design as shown. The dog tag is 2" x 1.1" in size and can be engraved on the backside (sorry, we do not offer an engraving service). This item also includes a 24 inch chain which can be easily shortened if desired.

The Do Not Resuscitate pendant and chain came promptly and is exactly as described. It is light weight, and when worn around the neck should alert paramedics and/or ER staff of the wearer's desire to refuse resuscitation. Caution: In some states this alert is not enough. An official form signed by the wearer and a physician is required to withhold resuscitation. I carry such a form in my wallet, and I had my pendant engraved on the reverse side as follows: DNR FORM IN WALLET.

Two weeks ago an acquaintance fell to the ground unconscious. The EMTs got his heart beating again, but he is brain dead. With no family and no written instructions, the state has kept that heart beating for two more weeks.

I have a will instructing "Do Not Resuscitate" here in my home, but what if I am traveling? Or shopping? Or in a theater?

This dog tag is always on: I shower, I dress up, I dress down, it doesn't matter. Nothing is worse for an EMT than to try to figure out what an unconscious patient would want. This tidy little dog tag eliminates any doubt. Amazon amazed me when I typed "DNR" into their Search box and this came right up!

Two BIG caveats: the chain is only plated. I thought I was becoming ill because my white shirts and night wear started showing a yellowish stain. I thought I was perspiring yellow! Instead, it was the plating wearing off. Now I have a dull metal chain. I guess I'll have to go to a jewelry store to buy a chain that isn't plated. Grrr..... The other problem is that the blue material is only glued and comes loose from the metal tag after less than a year of wearing it. Only buy this one if you can guarantee you are only going to live less than a year!

The EMT or emergency personnel will not look at your keychain for DNR info. Buy a bracelet instead since they are trained to look there first.

Bought this for my father at the beginning of December 2013. Today is Jan. 13, 2014. The tag separated into two parts--the metal and the blue part. My dad is now gluing it back together with epoxy. Not a big deal being such a small low cost item, but buyer beware...

The tag I ordered is made of good quality. No issues with it at all. Although this will not prevent a Dr or anyone else from resuscitating you in an emergency, it's a start. You also need to have a "sighed" DNR form from Dr on you (wallet or purse)

Yup. As the title suggests, this is what I wanted. But there's one thing I should say.

This is basically a plane dog tag with a thick plastic sticker on it. It's not painted. I bought it to wear in addition with the dog tags I already have. So I just pealed off the sticker and placed it on the back of my old dog tags. If I knew this, I might have tried to search just for the sticker rather than the whole necklace. Possibly could have saved a few bucks. But that's how it goes. I'm still happy to have it.

As an EMT, it was to my understanding that a DNR is only valid if the patient or patients family presents the official document signed. Otherwise an effort to save the patient will continue
