---Will it feel awkward?
Yes at first, and probably still will days and weeks later, BUT it's not actually a problem (for me anyway). It's just unfamiliar so each action takes a tiny bit extra time to move your hand the right way. You're re-learning muscle memory built up over years, so obviously it's not immediately natural. Now I find it very comfortable and natural.
---Is it ever a problem?
Not for the majority of stuff, but if I need really precise control (like selecting individual pixels in Photoshop), I switch back to a standard mouse for that particular action. I keep both on my desk so it's fine, but something about the change in grip and pressure (there's a lot less force on your fingertips with the vertical mouse, which is a good thing) causes a slight loss of precision.
---Does it really make a difference?
I've personally felt a lot less cramping in my hand and wrist. I routinely work really long hours on a project, and with a standard mouse, after a couple days of that my hand hurts. That hasn't happened nearly as much since I've been using this vertical mouse. I think it's definitely made things better.
Some things about this mouse in particular:
Maybe I have big hands (I don't know) but I find my fingertips extend well past the buttons and scroll wheel, so I'm really clicking the buttons with the middle-joint of my fingers. It's actually perfectly comfortable that way, but it was unexpected coming from a standard mouse.
For a while I left that base-plate extension thing on and it was fine, but I noticed the mouse was a little sluggish/bulky sliding around on the desk. When I took the base-plate off, the mouse felt much lighter and more nimble. So maybe try taking that off and see if you like it (it snaps on and off easily).
My only gripe about this mouse is the placement of the scroll wheel. Like I said my fingertips are well past the buttons, so they're also well past the scroll wheel. It's very hard for me to scroll with the middle of my finger -- I need to use my fingertip -- but the way I hold the mouse, I can't curl my finger comfortably and access the scroll wheel, so if I want to scroll, I have to change my hand position to get to it. It's maybe another of those slight awkward things that's actually a good thing (moving your hand more will likely lead to fewer cramps and carpal tunnel, etc.), but it's something I had to get used to.
Bottom line: This is a really inexpensive mouse that I'm very happy I bought. If you're thinking about a vertical mouse, I don't think you need one that's any more expensive than this.
UPDATE: THIS MOUSE SUCKS!
I have been involved or working with and around/in the IT field for about 15 years in some capacity and this mouse created an issue I have never seen before.
At first this was a great affordable alternative to the more costly Ergo mice out there. It is now less than a year old and it created a situation that at first had me thinking my PC was in need of new hardware. I was having constant low memory issues, rebooting was VERY slow and often hung, the BIOS failed to load and was recovered. (Gigabyte Dual BIOS). Once I was in the BIOS options the screen would filter in sort of like loading a webpage using a 56K modem. Since I built this PC some time ago and it runs pretty much 24/7 I first thought it must have a heat or hardware problem. Luckily neither was the case, this mouse was the issue. Again in all the years I have never seen such a thing. I have seen a bad USB cable cause a PC to hang on boot but this multiple problem situation was unusual to say the least. The PC is running great as it always has again now simply by unplugging this mouse. I do intend to email customer service and see what they have to say about this but even if they were to replace it I don't feel comfortable using it long term now. Also I will try to come back and update how customer service handles this situation.
UPDATE TO THE UPDATE!
After I edited my first review of this mouse and added the troubles it caused me I sent the seller an email. They responded VERY quickly and also issued a full refund in addition, couldn't ask for better support from a seller. The mouse still gets a 1 star for the issues it created but the seller gets a 10 for stellar support from the quick response to the hassle-free refund on the cost of the mouse.
