I don't have the 4th edition but this exercise book is so riddled with perfect errors that I have to wonder what the definitions in the master text could possibly be to get such terrible results. Perhaps there in an English teacher out there who can enlighten me. Almost always, an incorrect definition will spawn incorrect examples; bad examples flow naturally from bad definitions.
These authors are also prone to jamming, by which they start with a simple sentence like "Columbus discovered America". To make that past perfect, according to their method, all you need do is put 'had' in front of 'discovered, and voila, there you have a past perfect sentence: "Columbus had discovered America", which is not, as any sixth-grader can tell you, "past perfect".
