The VerticalMouse 4 (Right handed mouse) by Evoluent helps avoids forearm twisting for comfort and good health. The shape is thoughtfully sculpted for uncompromised comfort and easy-to-reach programmable buttons. No finger contortions are needed to operate the buttons. The thumb rest has been reshaped to provide even more comfort over earlier versions. Now featuring two thumb buttons, a new bottom thumb button provides greatly extended functionality when used with the included driver. An extra wide lip along the bottom edge prevents the last finger from rubbing the desk. Pointer speed controls on the side allow convenient adjustment without even releasing your grip from the mouse. Top mounted LEDs show through the chrome thumb rest to indicate selected pointer speed including extra low (XL), low (LO), MD (medium), to high (HI). Basic functionality in OS X or newer without a driver Linux - Basic functionality without a driver

But a little over a half year later, the second one started doing the same double-click thing. When I contacted Evoluent this time, they said that the warranty only applied to the original purchase date, and that was more than two years past, so they wouldn't honor it. The periodic double-click when I'm trying to single click makes the mouse too annoying to use.
It was good while it lasted, and it's hard to find a mouse for Macs anymore that allows you to program a double-click. None of the Logitecs support that anymore. But for the price of this one, I would hope to get a little more mileage out of it.
The finish is terrible. It gets soft just from your hand oils and starts rubbing off after a year or so. I saw another review that said that alcohol would dissolve the finish, so after it started coming off, I used some alcohol on purpose to remove all the loose finish. The slick plastic that is under it actually feels nicer to me than the original finish. I don't know why they felt the need to use such a terrible coating. It's not like there aren't finishes out there that can be in contact with hand oils and not dissolve.
2018 Update: It's still running fine many years later, just "naked" plastic now instead of a matte finish. So it's long lasting, other than the surface finish.
The software that comes with this is junk if you want to click and use the keyboard at the same time, at least under Windows 10. When I held down a key on the keyboard and tried to click on something, the click didn't go through. I first noticed it while trying to play a game, but it happens in productivity software too. I uninstalled the Evoluent software and switched to "X-Mouse Button Control" from highrez.co.uk to re-assign my buttons and never had that problem again.
A useful tip to turn off that obnoxious light on the back: make sure the computer is on, unplug the mouse, hold down the pointer speed "-" button on the side of the mouse and plug it in. No more light. It still shows the little green lights on the top to indicate the speed/sensitivity.
As a mouse it is good. Optical mouse, adjustable sensitivity, upright stance, puts the mouse cursor where you want it. It has outlasted the Anker wireless vertical mouse I bought around the same time whose scroll wheel has started becoming unresponsive. A bit expensive, but I'm hoping this and my trusty Microsoft 4000 keyboards will fend off carpal tunnel / RSI a bit.
Thank-you to Evoluent for thinking about those of us with disabilities.

It just does the job. I started using it straight away, and was able to get on with it immediately, no adjustment needed, so I think it is great.
The software to program buttons is as easy and obvious as it gets, and the extra buttons are really useful for CAD - I use SolidWorks, and had no problems adapting. My past mouse, Perixx MX2000 was great and I had esc delete and s keys programmed. With the Evoluent 4, I had to reduce this to esc, but it's not a huge loss. More buttons and a 2 way click tilting wheel would be even better, but it is very useful as it is.
The unique feature is the shape, and my immediate impression after working for a day is that my hand is in a lot more comfortable working position, no pain, which I would normally get after a few hours.
Apart from that, the usual quick delivery from Amazon.
Overall a pretty happy customer would definitely recommend. Will write again if anything changes.
In honesty i haven't tried any other vertical mice so I'm not sure what about it warrants the price tag, it doesn't feel especially premium everything is hard smooth plastic to the touch compared to say my Logitech G400s at home which has some really nice feeling surfaces.
If I had to pick any negatives i'd say its a little heavier than i'd like which doesn't help with achy fingers when doing precise mouse work and makes it especially unsuitable for gaming. Also the evoluent logo on the back is way too bright for a dark room.
Overall I don't regret the purchase, but I feel with £15 vertical mice around I find it hard to see this being worth 5 times the price.
I will definitely not be going back to a conventional mouse.
EDIT: After less than two and a half years, not enough for an expensive mouse, this mouse has had to be retired as the buttons are going faulty. First one, then a second and now a third button frequently register an unintended double click which, depending on what I am doing, can be very annoying. It doesn't appear to be possible to open the mouse up for cleaning without risking breaking it and reinstalling the software or changing button assignments makes no difference. I expect better from what was an expensive mouse, now much more expensive.
