The fan is reasonably quiet, but in the evening when the kids are in bed you can hear it.
Good things:
-- Cools my pi4 very well. (see above)
-- It provides decent access to gpio header (both from the top and from the side), has access slots for both camera and display cables.
-- clear top and bottom so you can see the board with alternating layers of black for "style"
-- I haven't tried it, but I bet you could combine 2 of these (you'll need to get longer screws) to allow for peripheral boards inside the case. (the benefit of the stacking layers)
Bad things:
-- documentation should have listed the pins to connect to, but it doesn't
-- you can't remove the microSD card without opening the case or using pliers/tweezers. If they had cut a small notch for you to get a finger to the card it might be easier. There is a hole the card can pass through, but you can't get fingers in it to pull the card out. Works fine to insert the memory card, and the clear bottom layer lets you see what you're doing.
-- very easy to get the individual layers misaligned or mixed up. I strongly suggest that if you are taking the case apart for connections or maintenance that you insert a couple of the screws opposite of what they say so they go up from the bottom. This will keep the layers aligned as you make adjustments.
-- The fan wire is easily trapped between layers while you're putting the case together. Takes a little patience to get everything in smoothly.
-- The fan doesn't stop if you simply use the raspbian shutdown menu item. You have to drop power to turn off the fan.
Other:
-- The opening in the case for power and video is "too tall" leaving lots of room for things to get into the case accidentally. I suspect is also leaves "lots" of room for larger connectors. Kind of a good/bad thing.
-- if you have a heat sink on your pi4 (like I did) from the official raspberry pi 4 case without a fan, odds are good it is too tall and will block this case's fan from spinning. Plan to remove that heat sink and use the one included with this case for your cpu -- it is short enough to work.
All in all, I'm quite happy with this case so far. I'll just keep my needle-nose pliers close by to pull the card if I need to rebuild it...
Update: the fan was mostly quiet at 5v, but I wanted quieter so I shifted to the 3.3v supply on pin 1. That is much quieter and only about 5C higher temperatures than with the 5v fan, still plenty cool enough. It went from idling at 50C to 55C, and a full 4-core compile (opencv) peaked at 70C, (it would have overheated and started throttling without the fan). So far so good!
Interesting idea, the design of this case. In the end I do wish I had bough a more traditional case design, as this case is much more trouble than it's worth if you have to open it up after assembling it.
To take the top off (which I had to do three times due to the fan just not fitting inside the case - it was hitting the CPU heat sink) you have to remove all four screws, and since those screws are the only thing holding the plastic plates that make up the case together, as soon as you remove the screws you have a structure that is no longer a structure and just wants to slide apart. Crazy frustrating, and since there is no benefit to having a case made up of 8 (or is it 10?) separate pieces plastic other than "it looks cool" I would just as soon had a normal case w/three pieces to it.
My CPU heat sink had come with a different case (the stock Pi 4 case) that didn't come with a fan, so the heat sink was taller than the one that comes w/this case. I decided I preferred to keep the larger heat sink, so I mounted the fan on the outside of the case rather than inside. Either way the fan has a slight whine and makes enough noise that when it's quiet you can hear it from 6 or more feet away. Just enough to be distracting and annoying when you're paying attention to it. I'm sure these cheap little fans vary in noise level depending on random chance, and maybe I've had the bad luck to get a louder one. You won't notice it if you're watching TV, listening to music, talking w/friends/family, but if the case is near where you like to sit quietly and read, it may become annoying over time. My awareness of it comes and goes, depending on how active I am.
Overall I would not purchase this again - too much hassle if you have to open the case w/the case pieces all wanting to go in different directions, and I'd take a chance on another brand hoping I'd get a quieter fan.

My recommendation for anyone who buys the product would be to test the placement of the two tiny heat sinks without removing the adhesive backing and make sure the acrylic layer fits over them properly before sticking them to the chips. If, like me, you screwed this up, you can snap the acrylic in half and remove the part that surrounds the larger of the two tiny heat sinks. It would help a lot if the manufacturer numbered the heat sinks and provided a photo indicating which numbered heat sink goes where - maybe for a future release?
It's also not clear from photos in the instructions which pins in the GPIO header you should plug the fan into. Of course, you can find this info online, but why not put it in the instructions? Red wire to pin 4, black wire to pin 6.
In the meantime, just be careful and the rest of the case is fine.
Just FYI to another review on here. The case is clear, but the outer layers come with a protective layer you need to peel off. Lastly, read the instructions before pulling this thing apart. Putting it together is easy once you do. It's a tight fit with the heat sinks so take a little care to position them correctly.
All works great and allows my Pi4 to be well ventilated. Great product.
