I used this device to take an HDMI signal from an AppleTV and to send the audio to an audio matrix (amp) powering speakers throughout my home. It works exactly as I hoped. The device takes a single HMDI signal input (AppleTV in my case), throughputs the entire signal (audio and video) on the HDMI out (to my TV) and then also sends the audio signal out via the RCA (red/white) jacks that I then connect to my audio matrix (amplifier).
This device is perfect if you have an older A/V receiver that does not have HDMI inputs, or as in my case, have a whole home audio matrix (HTD Direct brand) that accepts only RCA inputs.
I own three of these units (one for each AppleTV).
I reserve 5 stars only for products that are "absolutely perfect" in my opinion. This product could be better if it featured a way to mount the unit with a bracket or something. I have three of them in my A/V cabinet and since they are small and lightweight, they tend to get dragged around by the connected cables - it's hard to keep my A/V cabinet neat looking. But 5 stars for technical function.

This unit produces audio output for headphones while passing the HDMI signal through to the TV from my cable box. I did not notice any degradation of the signal and the audio is clear and with normal volume.
I have not connected the SPDIF to anything so I do not know how well it works in this mode.

This box works great.
My amp is ancient, but still works. I used to have several different types of audio (analog R/L coax, S/DIF coax, optical), and several forms of video (single-coax composite, S-video, RGB coax). The amplifier being from ... I think the 1990s ... has no understanding of HDMI. My TV, being from the early-mid 2000s, has a VDI connector so I had an HDMI-to-VDI adaptor on its VDI input.
Previously, since the amp doesn't comprehend HDMI, I had the various audio sources going into the amp, along with the low-quality video sources, and I could use the amp to simultaneously switch audio and video for those low-quality sources, but the high-quality video sources, video sources I had to connect to the TV's various inputs, and switch audio in the amp and video separately in the TV.
So, I finally gave up and bought an HDMI switch box, and where necessary, I'm getting boxes to convert against HDMI. In the case of this particular box, its input is the output of the HDMI switch, its HDMI output goes to the TV, and the S/DIF goes to the amp.
That arrangement works fine.

Worked exactly as expected. I used it to go between my Blu-Ray player and TV. I have an HDMI from my Bluray to the Image converter box. Then and HDMI out to my television, (which carries audio & video), and I used the RCA jacks out to my audio system for external speakers.

This does exactly what is advertised, allowing me to split off the audio from the HDMI signal and route it to my audio system. The sound quality through the system is terrific -- great for movies and concerts but overkill for things like listening to the news. Fortunately, I can still listen to TV's speakers if I don't want to fire up the whole system. My only criticism of the unit is that it's got cables coming out of two sides, making it somewhat ungainly. This is definitely something you'd want to keep tucked out of sight. It's a "set and forget" item (no buttons or switches that need to remain accessible) so this isn't really a problem.

The unit I received did not work. May have been a faulty unit, but I didn't have time to test another.

The unit I received never worked. I tried it with multiple input devices, monitors and cables and it never produced an audio signal via the analog or digital outputs. Ended up returning the unit and going with another product I found on Amazon at a cheaper price. I did not contact the manufacturer for help so I cannot comment on their customer support. I have had better luck with other products from the same company.
