I did discover that I can highlight the chapter headings and in this way get a sudo table of contents in the list of highlights. Obviousy this seems silly when the publisher should have provided a real table of contents. I hope this will be useful to others in making an informed buying decision. I purchased several books from this series and they all have the same issue.
For me it fails to get 5 stars as it it suffers from the lack of a table of contents or an index, and could use both. On my iPad, this book is 549 "pages" and not being able to go directly to a specific section is annoying - search is not an acceptable substitute, as for most search terms, too many results come up. (Yes, I know I can add tags, etc. but that isn't quite the same.)
The only negative thing, for me, about this book were the occasional formatting errors - while the book is still readable, the formatting errors are slightly distracting.
This book was well worth the money; however, I had problems at times with the male authors frequent reference to readers (photographers) as "she". It may have been better to use a different pronoun, like "they", if the objective was to be socially correct.
