As a full-time practicing emergency physician with experience with Dragon Medical 10.1 for PC, I would highly recommend staying away from MacSpeech Dictate. I was disinclined to believe the negative reviews, however, they are true.

Speed and Accuracy
MacSpeech Medical's speed and accuracy are significantly worse than Dragon Medical 10.1 for PC. The program inappropriately inserts periods into words. It sometimes reverses the spelling of common words, such as 'of'. (see example below)

I spent 3 hours with their support installing and reinstalling the software in the hopes of fixing the problem. Deleting user profiles and then creating new profiles. Training and retraining. None of their suggestions worked.

While the 30 day trial seems tempting the time and energy is not worth it. It took nearly 30 minutes with an agent to obtain an RMA number from their support tech.

I used several headsets, including the wired headset that comes with the product, the Plantronics Calisto bluetooth headset, and my own Nuance PowerMic II headset. All created the same errors I described above.

Operating System
I used OS Snow Leopard for this trial. The product does not support OS Lion. The company's reps told me they anticipate a new version coming out in 1-2 months, however given the products poor performance and their poor service I would not believe them.

I purchased this product directly from, and not from Amazon.

In summary, not ready for prime at all. I would stay away and will be reverting to Dragon Medical 10.1 for PC with Parallels.

I loved the MacSpeech Dictate medical with my Mac until I upgraded to Lion. At that point, it would no longer work in my electronic medical record, Allscripts Pro (which is on a windows server). Previously, I connected to the Allscripts Pro server using remote desktop on my Mac and it worked fine. Also the accuracy of the dictations decreased dramatically, even with simple non medical words in native Mac programs. Currently there is not an upgrade, nor even a new medical version for Macs. Dragon does not currently support the medical version for Lion and a warning pops up everytime I open MacSpeech letting me know it does not work well with Lion. There is a NON medical Dragon dictate version for Lion. I would recommend not purchasing the current Medical MacSpeech Dictate if you have Lion.

I am a practicing cardiologist. I found Macspeech Dictate Medical marginally adequate for my dictations. The error rate was annoying and resulted in a lot of extra work typing corrections. In addition, Nuance refused to issue updates to the product. I was able to use it nevertheless with all of my OS X updates all the way through Mountain Lion. Last year Nuance released Dragon Dictate Medical for Mac. I was reluctant to update given some of the bad reviews, but relented when I upgraded Mountain Lion to Maverick. It was way overpriced even with the discount given to prior owners of Macspeech Dictate Medical.

The bad news: Dragon Dictate Medical for Mac is as bad as previously mentioned. The cursor moves all over the place especially when trying to insert text in the middle of a document. The application locks up all the time requiring frequent restarts. I was so frustrated I was ready to send it back for a refund (bought directly from Nuance).

The good news: When it's working, the accuracy is much improved from Macspeech dictate. I rarely have to make corrections.

Unfortunately, the accuracy is not worth putting up with all of the application bugs. Again, I was ready to send it back, however, I discovered a fix for the application bugs. Fortunately, I did not uninstall Macspeech Dictate for Mac after installing Dragon Dictate Medical for Mac. I started both applications simultaneously and amazingly the bugs with Dragon Dictate Medical mostly disappeared. (It still has problems placing text without skipping the cursor around, but I fix this by using Macspeech Dictate in these instances, and Dragon Dictate for all other dictations).

So, if you have Macspeech Dictate, you can run it at the same time as Dragon Dictate Medical and avoid most of the application issues. If you do not have Macspeech Dictate medical, avoid Dragon Dictate Medical until Nuance fixes the bugs.
