It was not plug and play for me, but I found some great instructions and it works fine now, thanks to militarycac.com. I am using Mac OS 10.8.4.
First of all, just open your Keychain Access app (in the Utilities folder), insert your CAC and see if it shows up in the top left corner. If it does, you can probably skip directly to Step D below.
Here's how I did it:
A. Go to: [...] You will be following steps 5a and 6.
Follow instructions in 5a to import all the CAC certificates into the Keychain Access app. I'm not really sure this is necessary, or if just the next step (Step B) will work without doing this. (The DOD intermediate certificates are not visible in the keychain by default. However, the certificates are pre-installed on the Mac OS X computer.)
B. To install a CAC enabler, go to Step 6 on the militarycac.com page above. I chose the CACKey program, which downloaded a .tar file (click OK at the alert message about the file extension), which opened to become a .pkg, which you click to install the software.
(I chose CACKey because I have an Oberthur CAC)
C. Restart the computer.
D. Go back to Keychain Access app.Read more ›
The device itself is pretty slick. It's about the size of your average thumb drive (about an inch by a quarter inch by a half inch), and comes with a little loop that you can put on your average badge lanyard (so, presumably, you can keep it on the same thing you keep the CAC on). It doesn't seem especially _rugged_, and I do have concerns that it might get broken in the environments I use my laptop in, but at $30, I'm not real worried.
Ordinarily I would have used a PC-card based reader, but I use this on my Macbook Air, which only has USB. For this purpose, it's pretty useful.
I have Windows 8 and internet explorer 10. Everything worked without having to download extra software or update certificates.
I also like how small and unobtrusive it is. It fits neatly in a small pocket in my laptop bag.
I can't speak to durability as I only got this product yesterday. It is made of plastic. It does not seem particularly strong or particularly flimsy. It is about what you expect in most aftermarket hardware add ons. It does fold up nicely for storage and to protect the connector. In the field you would probably have to put it somewhere to protect it from dust. However, I think that is the case with most electronics.
If I had to come up with a con it would be lack of flexibility. It is set up to open right over the USB port, so you really can't use the adjacent ports at the same time. Something you sacrafice for the compactness and portability.
I can't recommend this card reader enough! Buy it, you won't be disappointed.

Its pocket size. No cord to deal with. Nice back up. Unfolding the device takes up a lot of space covering your other port access. Read morePublished 4 days ago by scott.aa

Small, portable, and most importantly it works. Works on an Army AGM image, drivers load automatically. Convenient to be able to just put it in your pocket and go.Published 11 days ago by Daniel Brennan
Works like a charm! Downloaded all the DOD certificates from militarycaccom and i'm all good to go. 15 minutes max of setup time. Very low profile for a card reader which I like.Published 12 days ago by Colleen
This works well and is able to read my CAC, the only problem I have is I have it on my keychain and some times it opens up in my pocket, I hope it does not break some time.Published 14 days ago by Laura
