Debra Burdick has written a gem of a book on mindfulness. As a guide for clinicians, the book has everything: no-nonsense, clear style; plenty of background info; tie-ins with research; excellent illustrations; and plenty of original experiential tools for introducing mindfulness and leveraging motivation and compliance. This is the kind of resource on mindfulness that you are not just going to skim through: you are going to copy and dog-ear and share this book with your colleagues and your clients. This just might be the last guide to teaching mindfulness that you buy. --Pavel Somov, Ph.D., author of Present Perfect, Reinventing the Meal and Anger Management Jumpstart

Thanks again for all the Help, Mrs. Burdick! I will provide updates to my progress with implementation now that I have keys to the entire process.
I wrote Amazon customer service. The response was, in part, "Due to publishers restrictions you cannot get reproducible forms for these books as these books are in Kindle format. Amazon is just a seller platform for these books, the best person who can help you get these forms is the publisher of these books."
I was surprised at how little help Amazon offered. Customer service did not even provide contact information for the publisher.
The publisher was much more helpful. The rep told me to do a search in the Kindle book for "download." That will take you to a link to their site. Once there you can establish an account and download the forms. It works.
