Purchased Aug 2013, worked until Sept 2015. Can't really complain for the price!. The problem was that the wire can twist inside the adapter causing some of the solder connections to come lose. The front pries off easily and the pc board pops out. I re-soldered the connector, added some "Crazy Glue" into the strain relief and filled the back of the adapter enclosure where the cable exits with hot glue. Problem solved.
