Ran through all hardware tests to rule out that its the card, from memory swap, new power supply and existing video card all works fine.
Sent card to PNY and they have a terrible RMA process. Submitting RMA is fine, but once they get it, don't expect much information other than "testing in progress". Asked an RMA question via RMA dashboard to get "Card is being tested" response. Terrible customer service and terrible quality on this card. Waste of money. Will never recommend PNY simply due to customer service and warranty repairs. Stay away unless you purchase a third party warranty.
GeForce Hotfix driver 344.80 corrects this issue for most
It's the best thing ever when it works BUT
Especially for 4k monitors you are going to want to use display port 1.2 for 2160 @60hz (Unless you monitor supports HDMI 2.0) and as of Nvidia 344.48 driver a lot of people are getting black screens on the log in screen from boot up. This is usually corrected after a number of restarts but be warned.
(Also turns out switching from display port to another output does not eliminate the start up issue every time)
Will probable be corrected eventually with a new driver release but this problem is months old

If your tired of playing sub 60Fps in games get this card and you will not be disappointed.
I'm running on a headless Ubuntu box, so can't comment on the video quality. But it's hashing MD5 50% faster than my previous GTX 680, so I can't complain!
