I love all of Dr. Garson's e-books on statistics, and this one is no exception. I work as a statistical consultant and have to keep updated on a wide range of statistical techniques and software. I frequently refer clients to Dr. Garson's website, which I refer to as a "statistical encyclopedia".
Survival analysis is tricky when you can't assume proportional hazards, and I find that this text explains the theory behind parametric survival analysis in an understandable fashion. This e-book is also a very practical guide to performing parametric survival analysis; he includes examples of code and output in both Stata and SAS. At the end are assumptions required for the analysis and FAQs.
This is definitely the place to start if you find yourself needing to actually perform an analysis, not just understand it from a textbook.
