The only 4 star rate was for the comfort. I have the largest rubber cover on mine because the smaller ones fall out of my ears. After having them in for a couple of hours, it is a little uncomfortable. Not the aching you get when you sleep with your ear folded over, but a little uncomfortable. Maybe that's part of the price you pay for in ear headphones.
I use these with my iPhone 4. The microphone/control functionality is just as good as the stock ear buds from iPhone. The only problem is that I'm told i sound muffled to people i'm talking to on the phone with the mic unless i've got it turned so the mic hole faces my mouth. The other issue is that you can't hear your voice through the ear phones, so that is a little odd, you only hear your voice through your head. The voice control works everytime i've used it.
I used a $75 gift certificate to purchase these through Amazon, so the net cost to me was $19. They were listed in SkyMall for around $130.
I highly recommend these. I would buy them again at $90 if i needed to.
Sound quality one phone calls is good, as well, and the integrated mic does a good job of picking up the voice. I've had no complaints from the opposite party about not being able to hear me. For the price, I think these are an excellent value.
Some people have complained about them breaking, but my experience has been that you have to be careful with these devices. It's too easy to catch the cord while you are roaming about. To address this, I am reinforcing the cable with Gorilla Glue where it attaches to the buds and plug. This has worked great with the iPhone sync cables.
