I purchased everything on Amazon in preparation for making these trips as comfortable as possible so reviews are super important. I hope this helps you in your decisions.
I've tried several other inflatable travel cushions, but the Therm-a-Rest Travel Cushion is both the most effective and most economical. In addition, the valve is by far the most robust I've encountered. Others have failed due to poorly designed valves.
I just placed my 4th order for this Travel Cushion, this time on the German Amazon site (turns out you can select an English option at center right on their banner) for a friend who recently suffered 3 fractured vertebrae in a freak bus accident. Sitting is a challenge, air travel heroic.
Recently I did economy flights to and from Australia using this Travel Cushion, and survived both 14 hour flights with no back pain, very nearly a miracle. I tried using two of them for both seat and back support but ended up using only one for seat support. In addition to the flights I used the cushion while driving over 3000 km in a van. Again, no back problems.
I ended up giving away both cushions, one in Australia, the other after returning. But traveling without this cushion is not an option, so I ordered a replacement that I plan on keeping.
I also own several of their stadium cushions, which I've used on multiple international flights. I find that on international flights, generally the seats have enough cushion on the seat, but the back of the seat has the familiar C shape, which provides zero lumbar support. A few of the stadium cushions stacked one on top of the other or slightly offset, made a world of difference on my last few flights. Two cushions allow you to independently adjust low back and mid back. Only drawback is you have to readjust often.
For flights that last less than four hours, I typically find the seats not much better than sitting on a piece of wood! Horrible. This causes SI, sacroiliac pain, which can cause pain running down your leg to cramping, hip pain, etc.
This pair of mats held together with included Velcro tabs creates an opening to keep the pressure off your tailbone. If you suffer from any kind of tailbone pain, this is the ticket. These pads are a bit thicker than the other ones that therm a rest sells, so you can get by with one for a lumbar support. I can't imagine anyone complaining these would not help on a long flight under your low back.
As to sitting on one, I found it a bit small and could feel pressure on my hamstrings in a short amount of time using at home on my office chair. I really think the larger Therm a rest cushion would be better for a seat. They are about 1 inch, and that seems to be just fine. The biggest difference is there much larger, so it distributes the weight better.
So, 4 to 5 stars as a lumbar cushion, maybe two to three stars as a seat cushion. Good relief from tailbone pain if that's your problem. The standard cushion doesn't do much there. If you Velcro the two of them together, they do not slide as your back traps the bottom cushion. Used as a lumbar pillow it's not going to move anyway.
No doubt you will probably have to adjust the air pressure multiple times, but in my experience that also redistributes your body weight, and can keep you happy for a few more hours. When you're sitting for 12 or 13 hours on an airplane everything helps! I hate flying internationally, but until they make a faster plane, or Scotty can beam me to my location we have to make the best of it.
If anyone knows of a better solution, I'm all ears. These cushions to me represent one of the best, lightweight options out there, and are not going to spring a leak ( hopefully) anytime soon. I have one cushion that is three years old and another that's two, and neither one leaks. These are well-made.

Auto-inflation is a great idea, actually i have to let some air out because it can be a little too firm.
Very strong construction and i'm still using every day on my office chair, and being a large guy at nearly 18st I have to say I'm really impressed:)
There are loops where two can be joined together to act as a lower back support and I'll be getting a second to do just that.
Buy with confidence - these cushions are worth every penny!
