update 3/28/2016: The manufacturer read my review (below) and reached out to me after I requested a refund from Amazon. The manufacturer offered to send me a replacement. I accepted, and while I was waiting for it to arrive, I examined the wires I was using to connect to the box.
The box comes exactly as you see in the picture, with two female plugs. I had purchased a long extension cord, female to male, and a short extension cord, male to male. My earphone cord plugs into one box plug, one end of the short cord plugs into the other, the other short-cord end plugs into the long cord and the male of the long cord plugs into my TV.
Close examination showed that the plugs on the short cord are different. To my embarrassment, I discovered that the short cord is for mono only. Hence, my review below.
Anyway, I purchased a stereo, male-to-male cord and, with that, the box works perfectly, both stereo and volume control.
My apologies to the manufacturer and to potential buyers who might have been mislead by my original review.

Out of the box: Not stereo, sound in one ear only (must be a bad connection); crackling in the single ear - another sign of poorly executed connection. From my experience and the mix of good and bad reviews about this product, I can only assume the maker builds them and then doesn't test the product before shipping. Sometimes they work and sometimes they don't. Quality control does matter.
Not very expensive, but definitely a throw of the dice.
