To connect this to Home Assistant was not obvious to me - I have HA running as a Linux virtual machine under Windows - "Home Assistant Core" and I could never get this to work with the HA Z-Wave JS add-on directly even though my VM (VirtualBox) engine did pass serial ports through to HA. So instead I ran Z-Wave JS outside of HA on Windows directly, and used the Z-Wave JS server wrapper to connect it to HA via IP. That has worked perfectly. If you run the server wrapper like I did you should know that you have to specify the external config file and put the four Z-Wave security keys there. I start the server with this command:
ts-node.cmd server.ts COM3 --host 192.168.1.133 --config zwave_config.js --port 3001.
Your options, of course, will vary as will the keys you put in the .js file. I post this because it took forever to figure it out. Most folks I gather run HA on dedicated hardware, but I had a very solid Windows 10 on a NUC with loads of capacity running other things that require Windows. By the way I also use Node Red running directly on Windows and it has also been very stable and a great way to have HA connect up to other physical resources on the Windows server.
Once this was all figured out it has worked great. I ran a USB extension cable so I could position the dongle for the best reception. Pairing has been much easier than when I was using Vera, the old home automation server I left behind.
I’m getting much, MUCH better range and speed than I was getting from the old SmartThings hub I had been using.

I’m getting much, MUCH better range and speed than I was getting from the old SmartThings hub I had been using.
it is very important that the distance from the hub and the device you’re pairing are fairly close to each other when you initially set it up.
After initial pairing, the distance can be further.
I have it integrated with Home Assistant. It is working perfectly.

* FYI, if you need to factory reset this thing (I did to clear out some 'ghosting' nodes after my own cfg screwup), googling around may tell you to find a control panel/software to do so. All you really need to do (if you have any idea what the following is), is this:
echo -ne “\x01\x04\x00\x42\x01\xB8” > /dev/ttyACM0
I chose Z-Wave over Zigbee for the superior compatibility.
So far, I've installed a few Z-Wave switches and a Z-Wave recessed door sensor and they're all working well.
The only caveat is that since I have my Home Assistant server in the basement, I had to run a USB extension cable from my server, at the bottom of my rack, to the top of the rack (3') to make sure that the signal makes it to my main floor.
But from the main floor, the Z-Wave switches that I have installed act as repeaters for my devices on the 2nd floor.
Very pleased with the performance. My old SmartThings hub seemed to have terrible Z-Wave coverage and was always losing devices. No issues at all with coverage from this stick!
Update 15.03.2023
