I remember a part in the book where she is sitting at a table with him, his lawyer, mother, and fiancee. The lawyer asks her "have you consummated the marriage", she is explained the meaning of the word and replies "oh that, many times".

River Lady I wonder if this is the book you're looking for. I think it's the last in a trilogy by Jude Devereaux. But it reminds me of what you described in your original post.

Don't think it's River Lady. That one, the h was somewhat obsessed with H and pretty much stalked him down. She was v. pregnant when her father forced the marriage on him.
