You can use plain paper. Personally, I usually carry the package and label into the UPS Store, and let them attach it inside one of those sticky bags they have. So, that's an idea as well.

I do a lot of shipping; I generally just print out labels on normal paper and then use clear shipping tape to attach them to the box.

lizzi, just put whatever address it's actually going to. If it's going to a PO Box, you don't want to put a home address. Your label should be something like:

John Whoever
PO Box 12345
Anyville, Anywhere 00001-2345
