Do you have to pay for the sleep app? I know with the older fitbit you got 2 free weeks then it was a charge?

I dont think you pay for the app. The device has that capability already on it. None of the reviews online mention paying extra for that function

No, you do not pay for the sleep app. The sleep wristband comes with your Fitbit and your sleep is tracked via your online dashboard.

If you mean the computer account vs. a phone app, you can get more advanced sleep reports - and other reports - if you upgrade to "premium" for 49.99 per year but basic sleep data does come with the free account. I think the phone apps are all free
